Comments:
A complaint investigation was conducted today regarding classrooms not maintaining ratios. Interviews were conducted, and documentation were reviewed. Based on the preponderance of evidence the complaint was deemed valid with regard to ratios. The complaint related violation and one non-related violation are identified in the Violation Notice. Complaint related: Yes
Description: Based on interviews with staff and documentation, the ratio for children from three years to the age of eligibility to attend public school, to five years were not being followed. (1staff to 10 children)
Evidence:
Interview with staff and documentation of CSR revealed the Preschool B classroom was at a 1:11 ratio at 12:11 on 01/03/2020.

Plan of Correction: We will ensure the staff are in ratios at all times.

Complaint related: No
Description: Based on observation, there is not at least 12 inches of space between occupied cots, beds, and rest mats.
Evidence:
Occupied cots in the Preschool, and PreK-A and B were not spaced 12 inches apart during this inspection.

Plan of Correction: We will make sure cots are spread apart 12 inches and remind staff at the Health and Safety training on 1/30/2020
