Comments:
An announced non-mandated record review inspection for the YMCA Ferrum Elementary After School Program at the Ferrum Y was completed on 01/10/2020. The inspector reviewed 2 staff records during the inspection today. The inspector arrived at 9:30 a.m. and departed at 12:00 p.m. Thank you for your time today. Violations:
Standard #: 22VAC40-185-210-A-4-a
Description: Based on record review and discussion with staff, the facility failed to ensure that within 6 months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children including but not limited to the topics stated in the standards.

Evidence: The record for Staff #2 has a documented program lead promotion date of 06/01/2019. Staff #2 obtain only 10 hours of training during the 6 months before being promoted.

Plan of Correction: Will work with the staff to get the necessary documentation/training to qualify for the position.

Standard #: 22VAC40-185-240-C
Description: Based on record review and discussion with staff, the facility failed to ensure that program directors and staff who work directly with children shall annually attend 16 hour of staff development activities that shall be related to child safety and development and the function of the center.

Evidence:
1. The record for Staff #1 had 2 hours of training documented for the annual training period.
2. The record for Staff #2 has 12 hours of training documented for the annual training period.

Plan of Correction: Staff will attended training.

Standard #: 22VAC40-185-340-D
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Evidence: Staff #2 and Staff #3 were the only 2 staff members present during the 01/08/2020 inspection; neither Staff #2 and Staff #3 are qualified as program leader.

Plan of Correction: Staff #2 will be qualified as program lead.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and discussion with the staff, the facility failed to obtain central registry findings within 30 days of employment.

Evidence: The record for Staff #2 has a documented hire date of 05/15/2018; the date on the central registry finding was 02/05/2019.

Plan of Correction: All requests will be made within 30 days.
