Comments:
An unannounced non-mandated monitoring inspection, as a result of a self-reported incident that took place on 11/20/2019, was conducted on 01/06/2020. Review of video footage and interviews were conducted during the inspection. Violations were cited as a result of the inspection and are noted on the Violation Notice. Violations:
Standard #: 22VAC40-185-340-F
Description: Based on review of video footage and staff interviews, the facility failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence: The video footage reviewed during the 01/06/2020 inspection shows that on 11/20/2019, Staff #1 left the Toddler Room and returned approximately 5 minutes later. An adult intern, that is not considered a staff member, was in the classroom during the time that Staff #1 was out of the room. The video footage reviewed that the children were on their individual cots and sleeping during the time that Staff #1 was not in the classroom. Staff #2 confirmed that there were 9 children, ages 16 months- 2 1/2 years, in attendance in the classroom during this time. Staff #2 confirmed that the intern in the classroom was not considered a staff member. There were 2 different views of video reviewed during the investigation; one was of the hallway where Staff #1 was observed leaving and returning to the classroom and the second was of the classroom were the children and the intern were visible during the time that Staff #1 was out of the room. During the approximate 5 minutes that Staff #1 was not in the classroom, the children were not within actual sight and sound supervision of staff.

Plan of Correction: Staff #1 was spoken to and given a written warning about interns are not allowed to be left alone with the children at any time and that children must stay in sight and sound supervision at all times. This was previously discussed at orientation.
