A Subsidy Health and Safety inspection was conducted on 1/6/2020 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Thirty-eight children were in care today with seven classroom staff and an administrator.
Subsidy violations were observed were observed and are listed on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on the review of seven staff files, the facility failed to ensure that all staff complete a TB test or screening within the last 30 calendar days of the date of employment and before coming into contact with children.
Evidence:
1. The TB test results for staff #1 are dated 2/25/19. The hire date was 10/30/19.
2. The TB test results for staff #3 are dated 10/18/19. The hire date was 9/30/19.
3. The TB test results for staff #4 are dated 11/1/19. The hire date was 10/28/19.
4. The TB test results for staff #5 are dated 10/17/19. The hire date was 8/22/19.
5. The TB test results for staff #7 are dated 3/19/18. The hire date was 8/13/19.

Plan of Correction: The assistant director will ensure that all staff have a TB test or screening within the last 30 days before employment and before coming into contact with children.

Standard #: 22VAC40-665-580-E-1
Description: Based on the review of seven staff files the facility failed to ensure that all staff complete first aid and CPR training within 90 days of employment.
Evidence:
1. Staff #3 was hired 9/30/19 and has not completed first aid and CPR training.

Plan of Correction: The assistant director will ensure that staff #3 completes first aid and CPR training as soon as a class can be scheduled.
