Comments:
The licensing inspector conducted an unannounced monitoring on Thursday, January 9, 2020 between 10:00 am to 1:15 pm determine the provider's compliance with licensing standards at the child day center. During this inspection, there were 26 children observed in the Children's House and Toddler Community and 5 staff present at the center including administrative staff. The licensing inspector completed observations of the children in care participating in a variety of centers, and interacting with peers and staff. The bathroom areas, classrooms, and equipment was inspected. The required posting were inspected. There were five children records and four staff records reviewed during this inspection. There are multiple staff with current MAT, FA/CPR, and Daily Health Observation training on site. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children.
Evidence: There was a key in the door knob of the storage closet containing multiple cleaners/disinfectant supplies, and paint materials with the labels "keeping out of reach of children" "warning" ir "caution" located in the hallway nearby the Children's house restroom areas during the inspection.

Exhibit #1 and #2 were photographs of the closet observation with the key in the locked and the opened closet containing the various hazardous substances.

Plan of Correction: Safety lock installed on knob.

Standard #: 22VAC40-185-540-A
Description: Based on review of emergency supplies and interview, the center did not ensure first aid kid included all required supplies.

Evidence: There was no triangular bandages observed in the available first aid kits during the inspection. Interview verified that triangular bandages were not available for review during the inspection.

Plan of Correction: Required supplies replenished

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records and interview, the center did not ensure to have a central registry finding within 30 days of employment or volunteer service.

Evidence: In Staff#2's record (date of hire:12/02/19), there was not a central registry findings found. Interview confirmed the documentation of central registry findings was not available for review during this inspection.

Plan of Correction: Child Registry pending

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records and interview, the center did not ensure to obtain a copy of the results of searches of an out of state central registry.

Evidence: In Staff #2's record (date of hire 12/02/19) and Staff#3's record (date of hire: 08/14/19), a copy of requests of results of out of state central registry searches was not found. Interview with staff confirmed documentation of request was not available for review during the inspection.
