Comments:
Thank you for your assistance and your time during this unannounced monitoring inspection. The visit was conducted on January 13, 2020 from 10:00 am through 1:05 pm. There were forty-one children in
the care of eight staff and one administrator. The total enrollment for the program was fifty-two children. There were six children records, eight staff records, medication and documentation for two children, evacuation drills, emergency supplies, health inspection, and fire inspection reviewed. Diapering procedures, hand washing, bathroom procedures, free play, circle time, reading, singing,outside play, daily schedule, transitions, nap time, bottle-feeding, and art were observed during this visit. The exceptions to compliance were noted on the violation notice.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and verification from administrator, there was not written information to
demonstrate that the individual staff member possesses the education and experience required by the
job position.
Evidence:
1. Reviewing staff #1, staff #2, staff #3, and staff #6?s records there was no written documentation
of education or job history.
2. Administrative staff verified that there was not written documentation or an application on file
that had that information.

Plan of Correction: All current staff will be required to provide documentation of education and job history to be added to their file. In the future all applicants will be given an application that includes questions regarding education and job history.

Standard #: 22VAC40-185-540-C
Description: Based on the review of the emergency supplies and verification from administrator, the facility failed to
maintain the minimum requirements of a first aid kit.
Evidence:
1. The first aid kits did not contain antiseptic cleansing solution/pads.
2. Administrative staff stated that they had taken the antiseptic cleansing solution/pads out of the
first aid kits.

Plan of Correction: Antiseptic cleansing solutions/pads will be purchased and added to each of the first aid kits.
