Comments:
An unannounced monitoring inspection was conducted today from 10:35am-1:30pm. There were 43 children (2-5yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside on the playground, playing educational games on the tablet, painting, and engaging with Legos, puzzles, and blocks. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-160-A
Description: Based on record review, 1 of 3 staff members did not submit documentation of a negative TB screening.

Evidence: Staff C's (date of hire: 10/8/2019) record did not contain documentation of a completed TB screening.

Plan of Correction: The employee will get it done today.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 4 children's records did not contain the following information:

Evidence: Child #2's (age: 4 years old) record did not contain documentation of a completed physical form.

Plan of Correction: I will talk to the parents and see how we can help them obtain the physical.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe, and operable condition.

Evidence:
1. A blue, plastic, children's wagon on the outdoor playground had a broken safety strap.
2. The metal climber on the outdoor playground had chipped paint.
3. The wooden climber on the outdoor playground had areas of chipped wood and peeling paint.

Plan of Correction: We will get in touch with the contractor.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 3 staff records did not contain documentation of a central registry finding within 30 days of employment.

Evidence: Staff C's (date of hire: 10/9/2019) record did not contain documentation of central registry findings.

Plan of Correction: We will send it off today.
