Comments:
An unannounced monitoring inspection was conducted today. There were 43 children in care, with a total of 9 staff members assigned to their direct supervision. Children's records, the physical plant, children's nutrition, classroom programming, classroom supervision, staff training, and background checks were inspected today.

The children were observed making snow, playing outside, creating art projects, listening to stories, and eating lunch. The children were served meatloaf, wheat bread, mixed fresh fruit, mashed potatoes, and milk for lunch today.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children's health records, it was determined that one record does not include documentation of a completed physical examination under the direction of a physician within 30 days of the child's enrollment.
Evidence:
Child D entered care on 6/24/19, however as of the date of this inspection on 1/16/20 there is no documentation of a completed physical examination.

Plan of Correction: The parent was called during the inspection and told to have the form completed today. If it isn't returned today, the parent was told that the child cannot return tomorrow.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record D does not include the employment location/contact information for each parent.

Plan of Correction: The parent was immediately called during the inspection.

Standard #: 22VAC40-185-350-E-4
Description: Based on observations, it was determined that one classroom of 4 and 5 year old children was temporarily operated beyond the allowable staff:child ratio of 1 staff:10 children.
Evidence:
There were 14 children in care in the Jungle classroom (age 4 and 5), with a total of 2 staff members assigned to their supervision. The Inspector observed one staff person leave the classroom for approximately 3-4 minutes, leaving the 14 children with only 1 staff person. While she was out of the room, the class was operated beyond the allowable ratio of 1 staff:10 children.

Plan of Correction: During the inspection the staff person was reminded to call for a floater to relieve her if she needs to leave the room. All staff will be reminded of the importance of maintaining staff:child ratios at all times.

Standard #: 22VAC40-191-60-D-1
Description: Based on a review of staff background checks, it was determined that management staff did not conduct the required follow up when one out of state background search was not received within 45 days.
Evidence:
The out of state central registry search request for staff person A was submitted on 11/15/19. The search findings were required to be received no later than 1/3/20. As of the date of this inspection on 1/16/20,. the search findings have not been received. Management staff did not conduct the required follow up to determine the delay in receiving the results.

Plan of Correction: I called the department this morning, but had to leave a message. The recording says that all search requests can take 5-8 weeks to be processed. I will continue to try to contact them. I will also keep documentation to show when I contact them and what the response is.
