An unannounced renewal inspection was conducted on 01/20/2020. There were 26 children, ages 4 years-12 years, and 5 staff members present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 1 medication during the inspection. The children were observed in the following activities: in organized large motor play in the gymnasium, during lunch and going swimming. The inspector reviewed the emergency preparedness plans, the staff and parent handbooks and the emergency supplies during the inspection. The inspector discussed injury reports and sharing space with the staff. Standard #: 22VAC40-185-60-A Description: REPEAT VIOLATION
Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child #4 did not contain the name and phone number of the child's physician.
2. The record for Child #5 did not contain the following information: the phone number of the child's physician, the health information as required by 22VAC40-185-150 and previous child day care attended by the child.Plan of Correction: The Child Care Coordinator will call the parents and get it corrected today.
Standard #: 22VAC40-185-240-B Description: Based on the review of policies and procedures and discussion with staff, the facility failed to ensure that staff shall be provided in writing with the information listed in 22VAC40-185-420-A and the policies/procedures required by the standards.
Evidence:
1. The following policy/procedures required by 22VAC40-185-420-A were not in writing: the center's transportation policy, the center's policy regarding application of sunscreen, diaper ointment or cream and insect repellent, the custodial parent's right to be admitted to the center as required by the Code of Virginia, the policy for communicating an emergency situation with parents and the appropriate general daily schedule for the age of the enrolling child.
2. The following required policies/procedures were not in writing: procedures for supervising a child who may arrive after scheduled classes or activities, including field trips, have begun and procedures for response to natural and man-made disasters.Plan of Correction: The handbooks will be corrected to include the policy/procedures.
Standard #: 22VAC40-185-420-A Description: Based on the review of policy/procedures and discussion with the staff, the facility failed to ensure that before the child's first day of attending, parents shall be provided in writing with the policies/procedures as required in the standards.
Evidence:The parent handbook given to parents at enrollment, before the child's first day of attending, did not contain the following policies/procedures: the center's transportation policy, the center's policy regarding application of sunscreen, diaper ointment or cream and insect repellent, the custodial parent's right to be admitted to the center as required by the Code of Virginia, the policy for communicating an emergency situation with parents and the appropriate general daily schedule for the age of the enrolling child.Plan of Correction: The handbook will be corrected.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and discussion with the staff, the facility failed to obtain a central registry finding within 30 days of employment.
Evidence:
1. The record for Staff #3 had a documented hire date of 11/14/2019; the central registry findings were dated 12/18/2019. Staff #3 was observed working during the 01/20/2020 inspection.
2. The record for Staff #4 had a documented hire date of 10/16/2019; the central registry findings were dated 12/06/2019. Staff #4 was observed working during the 01/20/2020 inspection.Plan of Correction: The Child Care Coordinator will document when not received within 30 days.
