63.2(17) License & Registration Procedures

Comments:
An unannounced renewal inspection was conducted on 01/21/2020. There were 30 children, ages 5 years-11 years, and 4 staff members present during the inspection. The inspector reviewed 5 children's records and 4 staff records. The staff stated that there is no medication currently on-site. The children were observed in the following activities: arriving to the program, during afternoon snack, during homework time, participating in an art project and preparing to go to the gymnasium. The inspector reviewed the emergency preparedness plans, the injury prevention plan, the playground safety plan, and the emergency supplies during the inspection. The inspector discussed annual training hours and personal items from home with the staff. The inspector arrived for the inspection at 2:30 p.m. and departed at 5:00 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Description: Based on document review and discussion with the staff, the facility failed to obtain an annual approval from the health department.

Evidence: The most recent health inspection was dated 08/16/2018. A follow-up with the health department was made on 09/09/2019.

Plan of Correction: The site director will discuss with the program administrator.

Standard #: 22VAC40-185-350-E-5
Description: Based on observations and discussion with the staff, the facility failed to ensure that the staff to child ratio for children from age of eligibility to attend public school through eight years, one staff member for every 18 children was met.

Evidence: Staff #4 was the only staff member present in the cafeteria as the children arrived to the program beginning at 2:40 p.m. At 2:42 p.m. there were 24 children, ages 5 years-11 years, present. Staff #2 and Staff #3 both arrived into the cafeteria at 2:44 p.m. The required staff to child ratio of 1 staff for every 18 children was not met until 2:44 p.m. when Staff #2 and Staff #3 arrived.

Plan of Correction: Come up with a different system to maintain ratio.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and discussion with the staff, the facility failed to obtain a central registry finding within 30 days of employment.

Evidence: The record for Staff #3 has a documented hire date of 11/13/2018. The date of the central registry finding was 12/27/2018.Staff #3 was observed working during the 01/21/2020 inspection.

Plan of Correction: The site director will discuss with the program administrator.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and discussion with staff, the facility failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.

Evidence: The record for Staff #3 has a documented hire date of 11/13/2018; the date on the employment eligibility letter was 11/19/2018.

Plan of Correction: The program director will discuss with the program administrator.
