Comments:
Today an unannounced Monitoring Inspection took place between the hours of 2:15pm and 3:45pm. There was 1 rooms observed with a total of 61 children with 4 staff within the supervision guidelines. The children were observed arriving from school, greeting staff, doing homework, coloring, painting, using melting beads and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, staff records did not contain all required information that is required to be kept on site.
Evidence: There is no documentation of emergency contacts available for Staff #3 and Staff #4. The site supervisor thought that the emergency forms should be kept in the programs internal system, but they could not locate the emergency contact information for these two staff.

Plan of Correction: I'll contact our main office and let them know that the emergency contact form for our new staff is not accessible in our electronic system. I'm also going to request to keep a paper copy of our staff's emergency contacts at our site.
