Comments:
An unannounced monitoring inspection was conducted this date. When the Inspector arrived, there were 44 children with a total of 9 staff on site. The Tulips class was observed in the large programming area free playing with staff interaction. A small group was observed returning to this room from the bathroom. Ratios were maintained during this transition. The Infant class was observed finishing up lunch and transitioning to nap time. Both diapering and hand washing procedures were observed. Hallway transitions have improved since the August inspection. Children were observed familiar with the procedures of lining up, sitting calmly in the hallway and awaiting their turn for the bathrooms. A total of 5 children's records and 4 staff records were reviewed. Medication on site was reviewed. This inspection began at approximately 10:30am and ended at 2:15pm. Standard #: 22VAC40-185-70-A
Description: Based on staff record review, one staff record was not complete.
Evidence: Staff #1's record did not include documentation of references and orientation training.

Plan of Correction: We will ensure that this documentation is placed in the staff record.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the diaper surface was no longer non- absorbent.
Evidence: The diaper mat located in the infant room had a crack in the middle of the surface and is no longer non-absorbent.

Plan of Correction: We replaced this immediately.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication authorizations on site, the center's medication policy was not followed.
Evidence: Two medication authorizations on site did not include the duration of the medication.

Plan of Correction: We will request the MD to include the duration of this medication.

Standard #: 22VAC40-185-510-G
Description: Based on review of staff records and observation of medication on site, staff has not obtained the required training for medication administration.
Evidence: The course completed by staff to administer medication does not include training on administering Auvi-Q medication.

Plan of Correction: We will obtain this training as soon as possible.

Description: Based on observation of the center, a findings of the most recent inspection was not posted.
Evidence: The August 2019 inspection was not posted.

Plan of Correction: We will able to locate the inspection. We will post today's violation notice.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on staff record review, a background check was not completed.
Evidence: Staff #3 was hired in August 2019. No documentation of a fingerprint search was found.

Plan of Correction: We will have this staff complete a new search and also call Office of Background Investigations regarding the original search request. We will also forward the search results to the Inspector within 10 working days.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff record review, one out of state background check was not obtained.
Evidence: Staff #1 was hired in September 219 and lived out of the state of Virginia in the past 5 years. Results of an out of state Central Registry search was not obtained.

Plan of Correction: We will request this background search and forward it to the licensing inspector within 45 days.
