63.2(17) License & Registration Procedures

Technical Assistance:
As discussed, please keep the Division updated with regard to future expansion.
Discussed the requirements of baby bottle labeling and storage and also storage of food.
Discussed age and stage behavior guidance.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 21 children with 5 staff on site. Observation of all 3 classrooms, bathrooms and outside playground was made. Review of on site medication was completed. A total of 4 staff records, 6 children's records, evacuation drills and staff training was reviewed. The children were observed eating lunch and engaged in individual activities such as pairing shapes, playing on the playground and sandbox. The center was found clean and organized and equipped with both Montessori and early childhood equipment. This inspection began at approximately 10:45am and ended at 2:10pm. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on staff record review, two staff records were not complete.
Evidence: Staff #2 and #4 are in lead teacher roles. Documentation of lead teacher qualifications was not maintained.
Documentation of two references was not found for Staff #4.

Plan of Correction: We will obtain copies of credentials and will forward to the Licensing Inspector for review. We will obtain two references for the one staff.

Standard #: 22VAC40-185-240-A
Description: Based on staff record review, one staff did not receive orientation training by the end of the first day of employment.
Evidence: Staff #4 was hired in August 2019. No documentation was found that this staff received orientation training.

Plan of Correction: We will provide this training and document the completion.

Standard #: 22VAC40-185-520-C
Description: Based on observation of the bathroom located off the toddler classroom, topical ointments were found accessible to children.
Evidence: Two diaper ointments were found located in a basket on the bottom shelf of a bench. This area was accessible to children in care.

Plan of Correction: We removed the ointments immediately.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation of lunch, food was offered that were potential choking hazards.
Evidence: Two children under the age of 3 years were observed eating large pieces of fruit that were considered potential choking hazards.

Plan of Correction: We will ensure that all food provided is cut up to prevent a choking hazard.

Standard #: 22VAC40-185-560-N
Description: Based on observation of lunch, one child was observed walking around while eating.
Evidence: One child was observed walking around their chair with a large section of banana in his mouth.

Plan of Correction: Staff reminded this child to sit while eating.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff record review, one background check was not obtained.
Evidence: Staff #4 was hired on 8/22/19. Based on the Sworn Statement, this staff has lived outside of Virginia during the past five years. An out of state Central Registry search request was not completed.

Plan of Correction: We will request an out of state search today and forward the search request and search results to the Licensing Inspector once received.
