Comments:
An unannounced monitoring inspection was conducted today from 9:45 am until 1:30 pm. There were 60 children present with 12 staff supervising. The children were observed playing with floor toys, playing with musical instruments and working on their shapes. The staff had very positive interactions with the children in their care. The physical plant, attendance logs, emergency plans, cribs, children's records, staff records, lunch boxes, emergency supplies and medications were inspected. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the center failed to ensure that children's records contain the required documentation.

Evidence:
Child #1's record does not contain documentation of the mother's place of employment.

Plan of Correction: I will request information from parent.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances shall be kept in a locked place using a sfe locking method that prevents access by children.

Evidence:
Hand sanitizer was not in a locked place using a safe locking method.

Plan of Correction: Removed during inspection.

Standard #: 22VAC40-185-500-B
Description: Based on review of diaper areas, the center failed to ensure that a nonabsorbent surface shall be used for diapering.

Evidence:
In the 2 1/2 year old classroom, there is a tear in the changing pad which renders it nonabsorbent.

Plan of Correction: I will purchase a new pad.

Standard #: 22VAC40-185-510-B
Description: Based on observation, the center failed to ensure that over the counter medication shall expire or be renewed after 10 work days and long term over the counter medication may be allowed with written authorization from the child's physician and parent.

Evidence
Child #7 and #9 had long term over the counter medications without a physician's authorization.

Plan of Correction: I will have parents fill out medication form with physician's signature.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that food brought from home shall be sealed and clearly dated and labeled with child's name.

Evidence:
In the Mobile Infant classroom, child #8's snack was not labeled with a name or date.

Plan of Correction: I will remind parents to label and date all food brought from home.

Description: Based on observation, the center failed to ensure that formula that is refrigerated shall be dated and labeled with child's name.

Evidence:
1) In the Young Infant classroom, child #6's formula did not contain a current date.

Plan of Correction: I will remind parents to label and date infant formula.
