22VAC40-185-210 - Reviewed program leader qualifications.

Comments:
An unannounced focused monitoring inspection was conducted on January 22, 2020 from approximately 9:45 am to 1:00 pm. Three staff were present with six children ranging in age from three to five years. Four children and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 2 of 4 children records has documentation of required information.

Evidence: The following was found during review of children records.
1. The record for child #1 did not have start date, mother's employment phone number and emergency contacts addresses.
2. The record for child #2 (start date December 9, 2019) did not have previous programs attendance of documentation of viewing proof of child's identity/age.

Plan of Correction: Staff will get the information from parents.

Standard #: 22VAC40-185-270-C
Description: Based on observation and review, facility did not ensure inside areas used by children are not lower than 68 degrees.

Evidence: Children were observed playing freely in a large room that is used as the gym. The thermostat reading showed the temperature to be 62 degrees.

Plan of Correction: The temperature will be increased when the room is used by children.

Standard #: 22VAC40-185-290-3
Description: Based on observation, facility did not ensure areas used by toddlers and preschoolers have protective covers are on the electrical outlets.

Evidence: Children were observed playing freely in a large room used as the gym. Five electrical outlets in the room did not have protective covers.

Plan of Correction: Staff will keep the electrical outlets plugged.

Standard #: 22VAC40-185-560-K
Description: Based on observation, facility did not ensure tables are sanitized before and after use for feeding.

Evidence: Staff was observed spraying the table with bleach water solution before serving the morning snack. The staff immediately wiped the bleach water solution with paper towels.

Plan of Correction: Staff will make sure they allow bleach water to air dry before wiping the table.

Standard #: 22VAC40-185-560-N
Description: Based on observation, facility did not ensure children are not eating or drinking while walking.

Evidence: Three children were observed walking while drinking water in the 4's & 5's classroom.

Plan of Correction: Staff will continue to stress to children to sit down to drink water.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review and interview, facility did not ensure an out of state central registry search is conducted when an individual has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #4 (start date January 9, 2019) had documentation of living outside the state of Virginia within the past 5 years and there was not a completed central registry search for that state. Owner stated a search had not been submitted.

Plan of Correction: Owner will call today to check into the process of completing the check.
