Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with 3 additional staff and 20 children. Four staff files and seven children's files were reviewed today. One child had medication on site.
Arrival time: 4:30pm
Departure time: 6: 25 pm

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation and interview, it was determined that there was no written record of children in attendance daily for the 11 children that were in the Library with the two staff.

Plan of Correction: We will make sure to keep a attendance log with each group of children.

Standard #: 22VAC40-185-190-B
Description: Based on a review of four staff files and an interview with staff, it was determined that there was no documentation in the file for one staff to show that this staff has management experience for their job position.
Evidence:
Staff A did not have documentation on file or at corporate office to show that she has 6 months of on the job management training.

Plan of Correction: We will put the documentation in the file.

Standard #: 22VAC40-185-510-F
Description: Based on a review of the medication and an interview with staff, it was determined that one child's medication did not have the original container or prescription label attached.
Evidence:
Child A's Auvi-Q did not have a pharmacy label attached to the medication.

Plan of Correction: We will get the original prescription label from the parents.
