Comments:
An unannounced monitoring inspection was conducted between the hours of 9:00 am and 11:00 am. At the time of the tour there were 44 children in care with 11 staff present. A sample of 2 staff and 5 children's records were reviewed, in addition to 2 medications. Children were observed playing with Legos, being read a story and watching a story on the television. School age children were present due to a school-out day.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
There were 6 wooden cubbies in the multipurpose room that were not secured, posing a tipping hazard.

Plan of Correction: The following corrective action was provided by the director during the exit interview.
The director will contact maintenance to have the cubbies secured in a way that they will no longer pose a tipping hazard,.

Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place, using a safe locking method.
Evidence:
The orange team classroom had an unlocked cabinet that contained the following hazardous substances:
bleach water and disinfectant wipes.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The cabinet was locked during the inspection. The director will remind staff to ensure all cabinets containing chemicals are kept in a locked place.

Description: Based on observation and interview, the center did not ensure substitute containers with hazardous substances in them, indicated the contents.
The orange team classroom had a spray bottle containing a liquid substance. Staff in the room stated the liquid substance was bleach water. The bottle was not labeled indicating its contents.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The labels have been coming off the spray bottles. A different label will be added to the spray bottle containing bleach water or a new bottle will be purchased and labeled

Standard #: 22VAC40-185-520-D
Description: Based on observation and record review, the center did not meet all requirements when insect repellent was used.
Evidence:
In the orange team's classroom, there were 2 bottles of insect repellent in a cabinet that were not labeled with the child's name.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The insect repellents were from the summer and are no longer in use. Both will be discarded and the director will retrain staff on the requirements when insect repellent is used.
