On December 19, 2019 the department received a complaint regarding supervision and records. The licensing inspector conducted an unannounced complaint inspection at the facility from approximately 9:00 AM until 1:30 PM on January 14, 2020. Interviews with staff were completed, observations were made and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
