Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and with regards of the center's process of accepting medication from parents into the program. As a reminder, please make sure that all authorizations for prescription medication reflect the information on the medication label.

Comments:
The following inspection took place between the hours of 2 pm through 3:30 pm. Today, I met with the Staff of the County of Loudoun, County After-School Activities (CASA) program held on site at Cool Spring Elementary located in Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 34 children observed with 2 staff; correct ratio for this one group. A total of 5 children files were reviewed. Staff files will be reviewed on a later date. The children were engaged in the following: check-in, bathroom break, hand washing, art, home work time, snack, both small and large motor skilled activities, and other organized activities. Violations:
Standard #: 22VAC40-185-510-G
Description: Based on review and interview, 1 medication indicated that the authorization for the center to administer a prescription to a child was not in accordance with the prescriber's instructions pertaining to dosage.

Evidence -
1. On the date of inspection (01/23/2020), an authorization to administer medication for Child #1 was reviewed.
2. The medication itself is dispensed in one full 0.30 mgs for each dosage. However, the physician's written authorization states that this medication should be dispensed as 0.15 mgs for each dosage.
3. Staff #1 stated (during an interview conducted on the date of inspection) that the Parent of Child #1 stated that the child's allergies has recently been diagnosed as more severe.

Plan of Correction: The medication authorization shall be returned to the parent for clarification. The doctor shall then make any needed adjustments.
