Comments:
A Subsidy Health and Safety inspection was conducted on January 23, 2020 to review supplemental health and safety requirements. Four staff were present with nineteen children. Additional staff were on site for kitchen and administrative duties. Four children and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on interview, facility did not ensure there is a current written list of all children's allergies, sensitivities and dietary restrictions that is dated and kept confidential in each room or area where children are present.

Evidence: There was not a list of children's allergies, sensitivities and dietary restrictions in the classrooms today and there was documentation of food allergies and dietary restrictions for at least two children. Owner confirmed there was not a written list at this time.

Plan of Correction: Owner will make a master copy of all allergies, sensitivities, and dietary restrictions and give a copy to all teachers highlighting the children for their room.

Standard #: 22VAC40-665-500-E
Description: Based on review and interview, facility did not ensure staff are informed of children's allergies, sensitivities and dietary restrictions.

Evidence: The following was found during tour of the facility and review of children records:
1. Staff stated in the infant/toddler classroom that there were not any children with food allergies or dietary restrictions. The record for child #1, that was present in the classroom, had documentation that child is lactose intolerant.
2. Staff stated in preschool classroom there was a child allergic to citrus, peanut butter and tree nuts. The record for child #4, that was present in the classroom, had documentation of egg, fish, peanut and tree nuts food allergy.

Plan of Correction: Owner will make a master copy of all allergies, sensitivities, and dietary restrictions and give a copy to all teachers highlighting the children for their room.

Standard #: 22VAC40-665-510-B
Description: Based on observation and interview, facility did not ensure for each group of children there is a written hard copy record of daily attendance documenting the arrival and departure of each child as it occurs.

Evidence: Three groups of children were observed and staff reported they did not have an attendance sheet for their groups.

Plan of Correction: For each group of children, staff will make sure they check in and out children on the attendance form as it occurs.

Standard #: 22VAC40-665-540-A
Description: Based on review, facility did not ensure 2 of 5 staff have a statement determining the individual to be free of communicable tuberculosis (TB) prior to coming into contact with children.

Evidence: The records for staff #2 (start date March 26, 2019) and staff #4 (start date April 26, 2019) did not have a TB test or screening.

Plan of Correction: No employees will be allowed to start without having a TB test.
