Comments:
An unannounced Monitoring Inspection was conducted today from 6:50am to 7:40am. There were 10 school-agers directly supervised by 2 staff. The physical plant, 1 staff record, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, socializing, playing Uno, washing hands for breakfast and eating breakfast. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review, the facility did not follow center policy for medication for parent or physician authorization.
Evidence: Reviewed the medication on site and found that Child 6 did not have documentation of parent or physician authorization.

Plan of Correction: Emailed parent of Child 6 and will return medication at pickup if paperwork is not brought in.
