An unannounced subsidy health and safety inspection was conducted on 01/21/2020 from 10:10 am - 1:00 pm. A monitoring inspection was concurrently conducted as this facility was a licensed child day center. At the time of entrance there were 70 children in care, ages infant - five years, with 12 staff. Two administrative staff were also present.
Children were observed during morning program time which included outdoor play. Records were reviewed for three staff and eight children. Nap was reviewed at the conclusion of the inspection.

Standard #: 22VAC40-665-540-A Description: Based on record review, the vendor failed to documentation of the TB screening shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff 1 has an employment date of 10/28/2019. The TB screening on file for staff 1 was completed on 02/07/2018.Plan of Correction: All staff whether new or transferring from another brand will have all applicable paperwork including TB test within 30 days of hire before they can start in the school. Management will inspect files periodically to ensure compliance. DM will check periodically to ensure files are in compliance.
