Technical Assistance:
Discussed locking the closet with art supplies on shelf.

Comments:
An unannounced monitoring inspection was conducted today beginning at 9:00am At the time of entrance, 45 children were in care with 3 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications, 5 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The children were observed playing games, reading and working on art projects. Interactions between the children and staff were positive. Standard #: 22VAC40-185-60-A
Description: Based on record review, one out of five records did not have complete information.
Evidence:
1. Child #4 (start date 8/28/2018) did not have documentation of the child's yearly update and confirmation of up-to-date information in the child's record. The last one was dated 8/28/2018.

Plan of Correction: I will call the parents today and I will have them review and sign the first day they return.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A bottle of lysol and disinfectant were found in an unlocked high cabinet near the refrigerator..

Plan of Correction: The cleaning products were moved to a locked cabinet during the inspection.

Standard #: 22VAC40-185-540-C
Description: Based on observation, the first aid kit is not complete.
Evidence:
1. The first aid kit was missing one triangular bandage.

Plan of Correction: I will purchase one and lock with the medications.

Standard #: 22VAC40-185-550-E
Description: Based on interview and record review, the center did not maintain a record of the practice drills for one year.
Evidence:
1. The center had a record of practice drills for each month, except the month of December 2019. Staff B stated the drill was completed, but did not get it recorded on the log.

Plan of Correction: I updated the log and will ensure it is documented monthly.

Standard #: 22VAC40-185-550-M
Description: Based on observation, the center's written record for a child's serious and minor injury did not contain all required items on the record..
Evidence:
1. Child A had a minor injury report dated 11/04/2019. It was missing the 2nd signature.

Plan of Correction: I will make sure to get a 2nd signature and remind staff.
