Comments:
An unannounced monitoring inspection was conducted today. There were 36 children in care, with a total of 3 staff members assigned to their direct supervision. Children's records, children's medications, staff information, staff training, emergency preparedness items, the physical plant, and child care programming interactions were inspected today.

The children were observed playing on the playground, engaging in free play activities, and preparing for lunch from home.
This inspection took place on a public school closure day.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that three records do not contain all required information.
Evidence:
1 - Child record B does not include "authorized pick ups" information.
2 - Child record C does not include: "authorized pick ups" information; each parent's work phone number.
3 - Child record F does not include each parent's work phone number.

Plan of Correction: Each parent will be asked for the information.

Standard #: 22VAC40-185-70-A
Description: Based on a review of on-site staff records, it was determined that one record does not include all required information.
Evidence:
There is no emergency contact information available on site for staff person C.

Plan of Correction: She will fill out the emergency contact form today.

Standard #: 22VAC40-185-240-D-4
Description: Based on staff interviews and a review of children's medications, it was determined that three children who have emergency medications on site were present without being in the care of a staff person who holds current Medication Administration Training (MAT) certification.
Evidence:
Of the three staff present today, only one holds current MAT certification. This staff person arrived at 9:45am today. It was reported to the Inspector that three children who have emergency prescription medications on site were present and in care prior to the arrival of this staff person. During the time prior to her arrival, these children were in care without a MAT certified staff person present.

Plan of Correction: We usually have 2 MAT staff, but the second one is not here today. I will make sure we are staffed correctly tomorrow.
