The crack in the cement on the preschool bike path needs to be addressed before it becomes a trip hazard.
Reviewed procedures for shelter-in-place practice drills.
An unannounced monitoring inspection was conducted on 01/21/2020 from 10:00 am - 1:00 pm. At the time of the tour there were 70 children in care with 12 teaching staff and two administrative staff. The children in care ranged in age from infant - five years. Morning program time which included outdoor play was reviewed. Records were reviewed for three staff and eight children. Nap was reviewed at the conclusion of the inspection.
One violation was observed and is listed on the violation notice.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and interview, the center failed to obtain a central registry finding within 30 days of employment.
1. There was no central registry finding on file for staff 1 who has an employment date of 10/28/2019.
a. Documentation on file indicates that a request was submitted on 11/06/2019.
b. Documentation additionally indicates that the program director contacted CPS to check on the results on 12/02/2019. CPS stated they had not received the original request and stated that it needed to be re-submitted.
c. The program director stated she had failed to re-submit the central registry check for staff 1.
Plan of Correction: This is in the process of being completed. All staff new and transferring in will have central registry sent on first day of hire. Management will follow up within 30 days of not receiving a Central Registry Check and document responses from the agency. Management will periodically check files to ensure compliance. DM will check staff files periodically to ensure compliance.
