An unannounced renewal study was conducted today. There are 46 children present at LI arrival with 5 staff. 5 staff records were reviewed for specific items only as the full record is reviewed at the main office at another time. 5 children's records were reviewed. See violation notice for missing items.Children were observed playing the gym and is the rooms, cleaning up from a cooking project, washing hands, eating snack and enjoying free time activities. Emergency supplies, emergency evacuation drills, shelter-in-place drills, first aid supplies and menu items served are all in compliance. LI was present today from 2:45 PM - 4:40 PM. Child 3's record does not include documentation of a completed physical. Date of Enrollment (DOE) 10/16/19.Plan of Correction: I will check with the school to see if it is on file, and ask the parent to provide a copy.
Standard #: 22VAC40-185-350-E-5 Description: Based on direct observation, the facility did not maintain a ratio of 1 staff member to every 18 children.
Evidence:
At LI arrival there was one staff member i the gym with 20 children.Plan of Correction: I am going to count the children before going to the gym. We will not be out of ratio.
Standard #: 22VAC40-185-420-E-3 Description: Based on a review of records, children's records have not been updated annually.
Evidence:
2 of 5 records reviewed were not updated annually.
*Child 1's record is dated 8/28/18.
*Child 2's record is dated 12/20/18.Plan of Correction: I will speak with the parents to have the record updated today.
