Comments:
An unannounced renewal inspection took place on January 27, 2020. There were 47 children in care from ages 4 years old to 11 years old with eight staff providing care and supervision. The inspector reviewed five children's records and five staff records on the day of the inspection. The inspector also reviewed three medications on the premises on the day of the inspection. This center does not transport children. Children were observed during the following activities: arrival, hygiene practices, snack time, and homework time. The inspector also reviewed emergency drill documentation, first aid kits, injury/incident reports, policies and procedures, and emergency supplies. There were discussions about the playground, staff records, and children's records. The inspection took place from approximately 1:45 PM to 5:00 PM. Standard #: 22VAC40-185-270-A
Description: Based on observation and discussion with staff, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include all elements as described by the standards.

Evidence:

On the playground, in three different locations, concrete used to support fence posts was exposed and above ground level which could pose as a trip hazard or other injury hazard.

Plan of Correction: The director will inform the principal and grounds staff about this issue.

Standard #: 22VAC40-185-330-B
Description: Based on observation and discussion with the staff, the facility failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

Evidence:

Several areas throughout the playground did not contain the required depth of resilient surfacing; dirt was visible around climbable equipment.

Plan of Correction: The director will inform the principal and grounds staff about this issue
