Comments:
An unannounced monitoring inspection was conducted on January 23, 2020 from approximately 10:40 am to 3:00 pm. Four staff were present with nineteen children. Additional staff were on site for administrative and kitchen duties. Four children and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 4 children records has a physical examination within a month of beginning attendance.

Evidence: The record for child #3 (start date January 21, 2019) did not have a physical.

Plan of Correction: Owner will get a copy from the parent.

Standard #: 22VAC40-185-160-A
Description: Based on review, facility did ensure 2 of 5 staff have a completed TB test or screening within 21 days of beginning employment.

Evidence: The records for staff #2 (start date March 26, 2019) and staff #4 (start date April 26, 2019).

Plan of Correction: TB tests have been done. Paperwork will be located and placed in staff files.

Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 2 of 4 children records have documentation of required information.

Evidence: The records for child #1 (start date September 10, 2019) and child #3 (start date January 21, 2019) did not have father's employment phone number.

Plan of Correction: Owner will get information from parents.

Standard #: 22VAC40-185-70-A
Description: Based on review, facility did not ensure 3 of 5 staff records reviewed did not have documentation of required information.

Evidence: The following was found during review of staff records:
1. The records for staff #1 (start date January 6, 2020), staff #3 (start date January 21, 2019) and staff #4 (start date April 26, 2019) did not have documentation of references conducted.
2. The records for staff #4 (start date April 26, 2019) and staff #5 (start date March 6, 2019) did not have documentation of orientation training although owner stated the training has been done.

Plan of Correction: All information will be documented, signed and placed in staff files.

Standard #: 22VAC40-185-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: The heat register along the side of the toilet in the hall boy's bathroom had yellow/brown speckled matter. Owner stated the yellow/brown areas were rust. The areas are accessible to children in care.

Plan of Correction: Owner will clean and repair, if needed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, facility did not ensure hazardous substances are kept locked.

Evidence: Paint, primer and laundry detergent were observed stored in an unlocked hall closet. The items were labeled "warnings or caution" and "keep out of reach of children".

Plan of Correction: Closet will be kept locked at all times.

Standard #: 22VAC40-185-540-E
Description: Based on observation, facility did not ensure the required nonmedical emergency supplies are available.

Evidence: The radio was not operable with batteries.

Plan of Correction: Owner will purchase new batteries.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review, facility did not ensure an out of state central registry search for child abuse and neglect was completed when an individual has lived outside of the state of Virginia within the past five years.

Evidence: The records for staff #3 (start date January 21, 2019) and staff #5 (start date March 6, 2019) have documentation of living outside the state of Virginia within the past five years and there was not a completed search of those state's central registry search.

Plan of Correction: Owner will call to follow up on the search submitted. Owner will go online to follow the requirements for the other states to have the searches done. Copies will kept of all submitted paperwork. Phone calls will be documented to include who was contacted, time, date and what information was received.
