Comments:
An unannounced full renewal inspection was conducted today. There were 46 children in care, with a total of 4 staff members assigned to their direct supervision. Children's records, administrative records, emergency preparedness items, the physical plant, children's nutrition, classroom interactions, and children's medications were inspected today.

The children were observed playing group games in the gym, eating lunch from home, playing games, enjoying free choice activity time, and making pancakes.

Violations:
Standard #: 22VAC40-185-150-A
Description: Based on a review of children's health records, it was determined that one record does not include a record of immunizations and a physical exam completed on the Virginia Department of Health form.
Evidence:
Child record A does not include an immunization record and a physical exam report recorded on the current Virginia Department of Health form.

Plan of Correction: I will ask the school nurse for a copy of the correct health forms.

Standard #: 22VAC40-185-50-A
Description: Based on observations, it was determined that the children's records are not maintained in a confidential manner.
Evidence:
The children's enrollment records and health records are stored inside a cubby that is placed beneath the staff desk in classroom A. These records are not locked and are not stored in a way to keep the information confidential.

Plan of Correction: The records have been placed in the cubby temporarily until they are filed in each child's record in the locking drawer. I will make sure the records are filed and all are stored inside the locking drawer.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that two records do not include all required information.
Evidence:
Child records A and C do not include documentation showing each child's date of enrollment.

Plan of Correction: The files will be updated.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that one area used by children has not been maintained in a safe condition.
Evidence:
The Inspector observed approximately 6 full size chairs stacked inside the children's bathroom in classroom B. Because of the height of this stack of chairs, it poses a safety hazard to the children if the stack of chairs were to fall on a child.

Plan of Correction: The chairs will be moved.
