The licensing inspector conducted an unannounced renewal inspection from 9:30 am - 12:00 pm. There were twenty four preschool children in care: eleven children with two staff in classroom #1 and thirteen children with two staff in classroom #2. A central office staff was also present. The children were observed in group activities, doing yoga, at circle time, listening and moving to music, and preparing for lunch. Four staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-60-A Description: Based on record reviews, the center failed to obtain all required information for the children's records.
Evidence:
1. The inspector reviewed five children's records. One of five records reviewed (child record #4) did not contain documentation of viewing proof of child's identity and age. Child's date of enrollment was 08/22/2019.Plan of Correction: A central office staff who was at the center, contacted the child's parent during the inspection. Staff reminded the parent that proof of identity and age is needed for the child's record. The parent said she will obtain a proof of birth record and provide a copy to the center.
