Technical Assistance:
Discussed the following with the Director:
- having an employee transfer to the center from another center under the same ownership
- background check timeframes for completion

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 6 staff files, 10 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 1:00-3:40pm. There were 100 children under the supervision of 15 staff. There are currently 8 staff certified in MAT (medication administration training). During the inspection, children were observed being fed lunch (infants), listening to storytime, reading books, cleaning up for and resting during naptime. For lunch, the children were served beef stroganoff, broccoli, honey dew, rolls and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Description: Based on file review, 1 out of 10 children's records did not have additional immunizations once every six months under the age of 2 years.
Evidence
1. Child #4 is 18 months old. The most recent immunization update was completed when the child was 10 months old.

Plan of Correction: We will ask the parents for this.

Standard #: 22VAC40-185-140-A
Description: Based on file review, 2 out of 10 children's records do not have a physical examination.
Evidence
1. Children #3 and #5 do not have physicals.

Plan of Correction: We will ask the parents for these.

Standard #: 22VAC40-185-160-A
Description: Based on file review, 1 out of 6 staff does not have a TB test/screening completed within 21 days of employment.
Evidence
1. Staff #6 began employment on 11/18/19 and does not have a TB test/screening on file.

Plan of Correction: Staff #6 has an appointment on 1/27/20 to have this completed.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 6 staff did not have all required paperwork on file.
Evidence
1. Staff #5 did not have documentation that two or more references as to character and reputation as well as competency were checked before employment began. There is one reference check on file.

Plan of Correction: A second letter of reference will be obtained to keep on file.

Standard #: 22VAC40-185-270-A
Description: Based on observations, there were areas of the center which were not maintained in a safe, operable condition.
Evidence
1. In the following classrooms (Early Pre-School, Pre-School Pathways 1, Pre-School, and Pre-K) there were areas where the paint was peeling off the walls.

Plan of Correction: I will have the maintenance person come in to fix this.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on file review, 1 out of 6 staff do not have the results of a search of the central registry by the end of 30 days of employment.
Evidence
1. Staff #6 has been employed at the center since 11/18/19 and does not have a completed search of the central registry on file. This was sent to be completed, but not within 7 days of having been hired.

Plan of Correction: Staff #6 will not work at the center until the results have been received.
