Comments:
The licensing inspectors conducted an unannounced monitoring inspection on Monday, January 13, 2020 to determine the center's compliance with licensing standards. The inspection was initiated at 10:54am and concluded at approximately 12:36pm. The census for today's inspection consisted of 14 children in the direct care of three staff members. Upon the inspectors' arrival, the children were observed in two separate areas of the building, playing in different centers. The children later transitioned into an art activity related to the theme of the week. The children were later observed taking bathroom breaks, washing hands, and eating lunch. Staff were observed having positive interactions with the children. The building is decorated with the children's artwork and is equipped with age and stage appropriate materials. The required posting were observed. The first-aid kit and non-emergency supplies were inspected. There were no medications reported on site. Five children's records were reviewed today. The emergency contact and related health concerns, if applicable for staff, were available and reviewed by the inspectors. Complete staff records are kept off-site and will be reviewed during the next inspection. The most recently documented evacuation drill was on 12/12/2019. The most recently documented shelter-in-place drill was on 12/02/2019.

Violations:
Standard #: 22VAC40-185-90--A
Description: Based on a review of five children's records and interview on 01/13/2020, the center did not ensure that the required written agreements between the parent and the center shall be in each child's record by the first day of the child's attendance.

Evidence: 1) The record of Child #1, enrolled on 08/21/2019, did not contain documentation of an authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds; a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. 2) During interview, a staff member reported the written agreements between the parent and the center were not in Child #1's file at the center and not available for review at the time of the inspection.

Plan of Correction: Per the Center: "The copy of the signed parent agreement form will be placed in the child's file tomorrow."

Standard #: 22VAC40-185-270-A
Description: Based on observations on 01/13/2020, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: 1) The licensing inspector observed the chain link fencing on the playground was separating from the fence posts in several areas, creating multiple sharp and jagged edges within the children's reach. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint, visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Plan of Correction: Per the Center: "Maintenance will be contacted today to work on repairs on the playground. Weather permitting, it will be fixed by next week."

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview on 01/13/2020, the center did not maintain the required non-medical emergency supplies.

Evidence: 1) Upon review of the center's non-medical emergency supplies, the licensing inspector observed that one working, battery-operated flashlight was not available on site. 2) Upon review of the center's non-medical emergency supplies, the licensing inspector observed that one working, battery-operated radio was not available on site. 3) During interview, a staff member reported the center's flashlight and radio were not in the building. The following non-medical emergency supplies shall be required: one working, battery-operated flashlight on each floor of each building that is used by children; and one working, battery-operated radio in each building used by children.

Plan of Correction: Per the Center: "A work order will be submitted to ensure batteries are purchased for the flashlight and radio. The items will be replaced within the next two weeks."
