Technical Assistance:
The Vendor said the crib available is not used and vendor was told no bumper pads and pillows are permitted for use.

Comments:
An unannounced was conducted from 1045- 1145. There were 2 children present with the vendor for a total of 4 points. The Vendor has First Aid and CPR certification, current TB screening, and current paper work for the children. The vendor does not have children with food allergies and is not MAT certified. Standard #: 22VAC40-665-160-B
Description: Based on interview, it was determined the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. Vendor said she has 4 kids enrolled and 2 were present during the inspection. The Vendor was asked for daily attendance and she said she does not take it. 2. Vendor said she uses the machine.

Plan of Correction: Inspector gave the vendor a blank copy if attendance to use.

Standard #: 22VAC40-665-250-A
Description: Based on observation, it was determined the vendor did not keep disinfectant out of reach of children. Evidence: 1. There were 2 children were present and these children have access to the bathroom. On the toilet, a can of Lysol disinfectant spray was observed.

Plan of Correction: I will lock it up.

Standard #: 22VAC40-665-410-A-3
Description: Based on record review and interview, it was determined the vendor did not practice the lockdown procedures in the last year. Evidence: 1. Vendor had written record of shelter in place drills and evacuation drills. The vendor had a check by the lockdown but when asked if she did it, she said no.

Plan of Correction: I will do a lockdown drill this month.
