63.2(17) License & Registration Procedures

Technical Assistance:
Technical assistance was provided regarding building maintenance, background checks, staff's orientation checklist and emergency drills.

Results of the out-of-state child abuse and neglect search must be sent to the LI within 10 work days.

Comments:
An unannounced monitoring inspection was conducted on this date from approximately 9:00 am to 1:00 pm. At the time of entrance, 131 children were in direct care of 32 staff members. Ratios were in compliance. Staff and children were observed during, circle time, tracing letters, cutting with scissors, completing sequences with blocks and outdoor play time. Infants were observed sleeping and crawling. Handwashing and diapering procedures were also observed. The site was clean, organized and contained a varied supply of developmentally appropriate materials. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, required posted documents, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-570-E
Description: Based on observation, it was determined that not all prepared infant formula was labeled with the child's name.
1) In the Infant room, "NIDO", 1 out of 2 bottles for Child 6 was not labeled with the child's name.

Plan of Correction: We will make sure all the bottles are labeled.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Central Registry search request within 30 days of employment.

Evidence:
1) On the date of inspection, the record for Staff 4 (start date 08/21/2018) did not contain documentation of an out-of-state central registry search request. Staff 4 had declared in their sworn statement having lived out of the state of Virginia.
2) On the date of inspection, the record for Staff 5 (start date 07/30/2018) did not contain documentation of an out-of-state central registry search request. Staff 5 had declared in their sworn statement having lived out of the state of Virginia.
3) Staff 2 stated that the out-of-state searches had not been requested.

Plan of Correction: We will submit the request immediately.
