63.2(17) License & Registration Procedures

Technical Assistance:
Tecnical assistance was provided regarding tummy time and background checks.
Results of the out-of-state Central Registry search must be sent to the LI within 10 work days.

Comments:
An unannounced monitoring inspection was conducted from approximately 2:00 pm to 4:40 pm. At the time of entrance, 58 children were in direct care and supervision of 10 staff members. Ratios were in compliance. Staff and children were observed during snack time, engaged in group activities, nap time, tummy time and crawling. The site was clean, organized and contained a varied supply of developmentally appropriate materials. A selection of staff and children records, the physical space, evacuation drills, attendance records and required posted documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, it was determined that the center did not obtain documentation of additional immunizations once every 6 months for children under the age of two years.
Evidence:
1) The record for Child 5 (dob 06/18/2018) did not contain documentation of a current immunization report. The most recent immunization report was dated 10/23/2018.

Plan of Correction: We will contact parents and request the immunizations.

Standard #: 22VAC40-185-240-A
Description: Based on record review, it was determined that staff did not receive training on recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by the Code of Virginia.
Evidence:
1) The records for Staff 1-3 did not contain documentation of having completed training on Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse.

Plan of Correction: I will hand out the information to staff and training will be completed as soon as possible.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Central Registry search request within 30 days of employment.
Evidence:
1) The record for Staff 2 (start date 09/03/2019) did not contain documentation of an out-of-state Central Registry search request. Staff 2 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.

Plan of Correction: I will submit the request immediately.
