Comments:
An unannounced renewal inspection took place on January 15, 2020. There were 49 children in care from ages 3 months old to 4 years old with 11 staff providing care and supervision. The inspector reviewed 5 children's records and 5 staff records onsite. The inspector also reviewed three medications and required documentation. This center does not transport children. Children were observed during the following activities: reading, coloring, painting, clean-up, dancing, snack time, outside time, and lunch. The inspector also reviewed emergency drill documentation, first aid kits, injury/incident reports, policies and procedures, and emergency supplies. There were discussions about the renewal process and children's records. The inspection was conducted from approximately 9:15 AM to 1:45 PM. Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to maintain and keep at the center a separate record for each child enrolled which shall contain all the elements as required by the standards.

Evidence:

1. The record for Child 1 did not contain chronic physical problems and pertinent developmental information and any special accommodations needed. The record for Child 1 did not contain previous child day care and schools attended by the child.
2. The record for Child 5 did not contain a complete address for the second person designated to call in an emergency if a parent cannot be reached.

Standard #: 22VAC40-185-280-D
Description: Based on observation and discussion with staff, the center failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food.

Evidence:

1. In the "3 and 4-year-old room," stored in a closet, bottles of soap, glass cleaner, and bleach were located above a package of rolls, a bottle of honey, and a container of plastic forks. The LI and Staff both observed and discussed this violation. Staff stated that they would relocate the food items.

Description: Based on observations, the center failed to ensure that each first aid kit shall be easily accessible to staff but not to children.

Evidence:

The LI asked Staff 5 where the first aid kit was located. Staff 5 showed the LI a first aid kit that was located in an unlocked and floor-level cabinet in the classroom. The cabinet is easily accessible to a small children.
