A Subsidy Health and Safety inspection was conducted on January 29, 2020 to review supplemental health and safety requirements. One staff was present with sixteen children at the onset of inspection. A second staff arrived during inspection. Five children and two staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-520-B Description: Based on review, facility did not ensure 1 of 5 children records has documentation of required information.
Evidence: The record for child #3 (start date September 3, 2019) had documentation of a food allergy and the record did not contain an allergy action plan.Plan of Correction: Staff will contact parent to get a allergy action plan from the doctor.
