An unannounced renewal inspection was conducted today from approximately 7:30am-9:10am. There were 17 children in the direct care and supervision of 2 staff. A sample of 5 children?s records and 2 staff records were reviewed. The children were observed entering the facility, eating breakfast, serving the children, and reading books. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-185-160-C Description: Based on record review, a staff member did not submit a tuberculosis screening at least two ears from the date of the initial screening.
Evidence:
The tuberculosis screening of Staff A was last completed 08/23/2017.Plan of Correction: Staff will obtain TB test.
Standard #: 22VAC40-185-40-H Description: Based on record review, staff could not provide proof of current liability insurance.
Evidence:
The liability insurance on file expired 10/31/2019.Plan of Correction: File will be updated.
Standard #: 22VAC40-185-260-A Description: Based on observation, the center could not provide proof of an annual fire inspection.
Evidence:
The last fire inspection was complete 12/07/2017.Plan of Correction: Staff will call for fire inspection again. That office is backed up.
