63.2(17) License & Registration Procedures

Technical Assistance:
The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 2:40pm and 4:30pm. There were 3 classrooms observed with a total of 30 children with 4 staff within the supervision guidelines. The children were observed playing house, coloring, socializing, playing freely, eating snack and going home with their parents. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, the program has not obtained documentation of subsequent tuberculosis (TB) test/screening results every two years.
Evidence:
1. Staff #2's most recent TB screening is dated 8/19/17.
2. Staff #4's most recent TB screening is dated 11/17/16.

Plan of Correction: I'll give them the forms and they'll have to contact their doctors.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records do not contain all required information.
Evidence: Child #2 file does not contain a second emergency contact.

Plan of Correction: I'll ask her mom.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records do not contain all required information.
Evidence:
1. Six out of six staff records do not contain address, verification of age requirement and job title.

Plan of Correction: I'll coordinate with the business office to get this information in our staff files.

Standard #: 22VAC40-185-240-A
Description: Based on record review, staff have not received orientation training by the end of their first day assuming job responsibilities.
Evidence: There is no documentation of Staff #4 and Staff #6 receiving orientation training.

Plan of Correction: I'll give them the form to complete.

Standard #: 22VAC40-185-240-C
Description: Based on record review, the staff who work directly with the children have not obtained a minimum of 16 hours of annual training related to child safety and development.
Evidence:
1. Staff #1 has attended 7 hours of training in the last year.
2. Staff #3, Staff #4 and Staff #5 files do not contain any documentation of training. All staff have been employed for more than 1 year.

Plan of Correction: Some of the staff have attended training's but they don't receive certificates at the end of the training's. I'll remind them that they need to obtain documentation that they attended.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review, the program did not obtain a subsequent sworn disclosure statement every give years for one employee.
Evidence: The most recent sworn disclosure statement for Staff #4 is dated 9/10/14.

Plan of Correction: I'll give her a copy to fill out.
