Comments:

Today an unannounced Monitoring Inspection took place between the hours of 10:10am and 1:15pm. There were 4 classrooms observed with a total of 34 children with 7 staff within the supervision guidelines. The children were observed working on worksheets for the number '18', practicing writing the letter 'S', walking through the halls for bathroom breaks and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations:

Standard #:	22VAC40-185-160-A
Description:	Based on record review, each staff member did not submit documentation of a negative tuberculosis (TB) screening that has been completed within 12 months prior to employment. Evidence: Staff #6 (date of hire: 8/12/2019) TB screening is dated 2/16/2018.
Plan of Correction:	I'll ask the employee to do a walk-in appointment to the medical clinic next door.
Standard #:	22VAC40-185-210-A
Description:	Based on record review and interview, program leaders did not meet the minimum qualifications. Evidence: Staff #7 is currently the program leader of the Pre-K 4's classroom. Staff #7 does not have a completed degree in a child related field or any programmatic experience prior to employment at the center. Center management has confirmed that Staff #7 is currently taking child related classes at a local community college.
Plan of Correction:	I'll make sure they complete the 24 hours of training to qualify for the position.
Standard #:	22VAC40-185-420-C
Description:	Based on observation, parents are not permitted to enter the child day program. Evidence: At the front entrance of the child day program there is a sign on a portable wall that states "Parents are not permitted beyond this point!" Parents are not permitted into the school hallway or classrooms.
Plan of Correction:	I'll take the sign down.
Standard #:	22VAC40-185-500-B
Description:	Based on interview, diaper changing surfaces are not cleaned with soap and water and sanitized after each use Evidence: The staff in the Pre-K 2's classroom stated that they wipe the diaper changing surface with a baby wipe after each child is changed.
Plan of Correction:	We'll do it immediately.
Standard #:	22VAC40-191-60-C-2
Description:	Based on record review and interview, the center has not obtained a central registry background check for each employee within 30 days of employment. Evidence: Staff #5 (date of enrollment: 10/1/2019) file does not contain central registry background check results. There is a copy of the central registry background check that was mailed out on 10/10/2019. Center management confirmed that they have not yet followed up with the appropriate agency to inquire about the background check results.
Plan of Correction:	I'll call the appropriate agency today.
