Comments:
A Subsidy Health and Safety inspection was conducted on 1/23/20 from 10:40am until 1:20pm to review supplemental health and safety requirements. At the time of entrance, there were 40 children in care with 10 staff present. 29 school-age children arrived at the center with 2 staff at the conclusion of the inspection. A sample of 5 children's records and 5 staff records were reviewed. Children were observed playing on the playground, engaging in morning learning activities, eating lunch and resting quietly during nap time. Hand washing, lunch service and school bus procedures were also observed. Medication, first aid and emergency supplies, documentation of emergency practice drills and children's injury reports were reviewed. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on record review and interview, the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The record for staff 1 (date of hire: 10/12/19) contains documentation of a negative TB test that was dated 7/27/19, which is more than 30 days prior to employment.
2. The vendor confirmed that the TB test for staff 1 was not completed within the required time frame.

Plan of Correction: The vendor responded with the following: A new TB test will be administered for the employee in question.

Standard #: 22VAC40-665-580-E-1
Description: Based on record review and interview, the vendor did not ensure that all staff have current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Evidence:
1. The records for staff 2 (date of hire: 5/8/19) does not contain documentation of current certification in CPR.
2. The vendor confirmed that the record staff 2 lacks documentation of current certification in CPR.

Plan of Correction: The vendor responded with the following: The employee?s in question needing First-Aid and CPR will be given a week to get certified,
if this is not done they will not be allowed to work until it is completed.

Description: Based on record review and interview, the vendor did not ensure that all staff have current certification in first aid.

Evidence:
1. The records for staff 2 (date of hire: 5/8/19) does not contain documentation of current certification in first aid.

Description: Based on a review of emergency drill records and interview, the vendor did not ensure that lockdown procedures shall be practiced at least annually.

Evidence:
1. The emergency drill record provided had no lockdown drill documented for 2019.
2. The vendor confirmed that a lockdown drill was not conducted in 2019.

Plan of Correction: The vendor responded with the following: A lockdown drill and shelter in place took place on 1/24/2020.
