Comments:

A Subsidy Health and Safety inspection was conducted on Wednesday, January 29, 2020 from approximately 10:00 am until 2:30 pm to review supplemental health and safety requirements. The center?s administrator was present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 44 children in the direct care of 16 staff members. Additional staff were on site and available to the children during the inspection. Upon arrival, the children were engaged in a variety of activities. Each classroom was inspected and formal observations were completed. The children were observed playing on the playground, working in centers, and during a group activity. The center is equipped with age and stage appropriate materials and equipment for the children?s use. Staff were engaged with the children and were observed offering guidance when needed. All areas where the children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. During the inspection, five children?s records and five staff records were reviewed. Medication is administered and four medication were reviewed today. The most recent evacuation drill was documented on 01/16/2020. A shelter-in-place drill was documented on 11/09/2019. The most recent lockdown drill was documented on 07/05/2019. Fire Inspection: 01/10/2020 Health Inspection: 12/16/2019 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental inspection and is identified on the violation notice. Description:	Based on a review of five staff records on 01/29/2020, the vendor did not ensure two staff were evaluated by a health professional and were issued a statement that the individuals were determined to be free of communicable tuberculosis (TB) within the required time frame. Evidence: 1) The TB results in the record for Staff #1, hired on 08/30/2019, were dated 09/03/2019. 2) The TB results in the record for Staff #2, hired on 09/23/2019, were dated 08/19/2019. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Plan of Correction:	Per the Vendor: Moving forward, we will ensure TB results are submitted at the time of hire.
