Comments:
The licensing inspector conducted an unannounced renewal inspection from 9:00 am - 12:30 pm. There were thirty eight children in care: eight infants with two staff, seven toddlers with two staff, eight 2 and 3 year olds with two staff, ten 3 and 4 year olds with one staff, and five preschoolers with one staff. The director was also present. The children were observed at circle time, singing, playing on the playground and during lunch. Five staff's records, two board officer's records and five children's records were reviewed. Areas of non-compliance are identified on the violation notice. Description: Based on observation and measurement, the center failed to have 6 inches of resilient surfacing in areas on the preschool playground.
Evidence:
1. The inspector observed the preschool playground. There was approximately 3 to 4 inches of resilient surfacing (mulch) surrounding the mats at the base of the climber. Fall height of the equipment is approximately 4 feet.

Plan of Correction: Staff will rake additional mulch from under the slides into these areas. Additional mats and mulch will be purchased and placed in these areas on the playground. Staff will monitor the resilient surfacing to ensure it remains at 6 inches in depth.
