Comments:

An unannounced renewal inspection was completed on 1/30/2020. There were 108 children present with 16 staff directly supervising, a kitchen staff person, and two administrative staff present. All policies and procedures were reviewed. Ten children's records and ten staff records were reviewed. One daily medication and three emergency medications were reviewed. The inspector arrived at the center at 9:35 am and departed at 3:00 pm. The children were observed during free play times, during structured activities, during TV time, during story times, during lunch, and during nap time. All infant activities were observed.

Standard #:	22VAC40-185-280-B
Description:	Based on observation, the center failed to ensure that hazardous substances were kept in a locked place. Evidence: 1. There were two aerosol air freshener sprays observed in two classrooms and three aerosol air freshener sprays observed in another classroom unlocked. The sprays in both classrooms were observed to be on top of the paper towel dispenser above the counter with the sink. The paper towel dispenser was approximately five feet high on the wall and not accessible to the children. The children observed in the classrooms were toddler age (17 months up to two years old) and infants (four months through 15 months). All of the air fresheners had "Keep Out of the Reach of Children" and other cautions/warnings on it which would require the sprays to be locked.
Plan of Correction:	This was corrected during the inspection. All of the sprays were locked in a cabinet with other cleaning supplies.
Standard #:	22VAC40-185-280-G
Description:	Based on observation and interview, the center failed to ensure that hazardous substances not kept in the original containers, have substitute containers that clearly indicate their contents. Evidence: 1. A generic, clear spray bottle was observed with a pink liquid in it that was locked with other cleaning supplies. The bottle had "cleaning sol" labeled on it. The staff was asked of the contents and stated that they were unaware of the actual name of the cleaner but it is the cleaner supplied from a supply company in bulk that has to be mixed when dispensed. The bottle did not clearly indicate the contents of the substance in the spray bottle. 2. There was a generic, clear spray bottle observed locked in a cabinet that was labeled "floor cleaner." The label did not clearly indicate the contents.
Plan of Correction:	The director will contact the operations staff person to get the actual name of the cleaner for all bottles to be labeled with the contents.

Description:	Based on observation, the center failed to ensure that children's hands were washed after diapering. Evidence: 1. There were two children observed to have their diapers changed (nine months and eight months) and the children's hands were not washed after being changed.
Plan of Correction:	The director will remind staff about proper hand washing procedures.
Standard #:	22VAC40-185-500-B
Description:	Based on observation and interview, the center failed to ensure that the diapering process was practiced as required. Evidence: 1. Two diaper changes were observed for children ages eight and nine months old. The diaper changing pad was not cleaned and sanitized as required. The diaper changing pad was not cleaned or sanitized after one diaper change and was cleaned only after the second diaper change. The Standards require that the diaper changing pad is cleaned and sanitized after each diaper change. 2. Staff was asked about the procedure after diapering and verified that the diaper changing pad was not cleaned after one child and was only cleaned after the second child.
Plan of Correction:	The director will remind staff of proper diapering procedures.
Standard #:	22VAC40-185-560-K
Description:	Based on observation, the center failed to ensure that tables and high chair trays were sanitized before and after use for feeding. Evidence: 1. Three high chairs were observed to be used. The high chair trays were cleaned with soap and water after each use. The high chair trays were not sanitized after each use. One high chair was observed to be used by a second child after being cleaned with soap and water and had not been sanitized.
Plan of Correction:	The director will remind staff of proper high chair sanitizing.
