We discussed the requirements for behavior guidance. The risk assessment for a violation was provided.

Comments:
During the unannounced inspection conducted on November 25 2019 from 11:04 AM to 4:10 PM, an investigation was conducted regarding the self-reported concerns about an incident and behavior guidance. After completing interviews on November 25, and 26, 2019 and on December 4, 2019, and a review of documentation and records, it was determined that the concern was valid. Standard #: 22VAC40-185-400-B
Description: Based on interviews conducted on November 19, 25, and 26, 2019, a review of pertinent records, center procedures and documentation, as well as observation of the classrooms and care of children, the center failed to ensure that staff always use behavioral guidance that is constructive, age appropriate and intended to redirect children to appropriate behavior to resolve conflicts. Evidence: On the morning of November 18, 2019, staff member 2 provided light-up balls to children in classroom 9 prior to the transition for outside time. Interviews confirmed that child 1 hit more than one child with a ball during play. Per interviews with staff and verification of staff member 1's history of past communication methods, staff member 1 was addressing the use of the balls with three children and had physical contact with child 1's face while using one hand to move child 1's head to face hers, as she was at child 1's eye level, and using an angry tone of voice. Staff member 1 turned and stopped speaking to the children when she saw staff member 3 at the hall door.

Plan of Correction: Staff member 1 is no longer employed at the center. The administrative staff will make sure all staff understand the center's discipline policy. Administrative staff reviewed appropriate teacher language, positive redirection techniques and having age appropriate expectations during a meeting with staff. Also, a review was conducted on how to recognize one's own feelings and ask for support from other adults in the building.
