The inspector conducted an unannounced complaint investigation on 01/24/2020 from 2:40 p.m. until 3:30 p.m. concerning a complaint received in this office 01/21/2020 concerning emergency preparedness. Interviews were conducted with staff and documentation was reviewed. The preponderance of evidence gathered during the investigation supports the allegations, so the complaint is determined to be substantiated. Two violations concerning ratios not being met in the classrooms were not reported but found during the inspection so violations are cited for being out of ratio. An intensive plan of correction meeting has been scheduled at the Western Regional Dept of Social Services office for February 11, at 10:00. Standard #: 22VAC40-185-350-E-2 Complaint related: No Description: Based on observation and interview, the center did not maintain the required ratios in the two year old room.
Evidence:
1. The inspector observed 13 two year olds and one 3 yr. old from approximately 2:50 p.m. until 3:00 p.m. with one staff person supervising. Staff #4 stated that the other staff person was on break, and the phone could not be found to call administrative staff to ask for assistance. The Inspector notified administrator that the class was out of ratio and a staff person was sent to the classroom.Plan of Correction: Not available online. Contact Inspector for more information.

1. The inspector observed 15 children in the preschool classroom with one staff person present. Eleven children were awake and four were asleep at approximately 2:55 p.m.to 3:05. Staff #5 stated the other staff person was on break. 2. The inspector notified the administrator the classroom was out of ratio and the second person had arrived back from break and went to the classroom.Plan of Correction: Not available online.
