Technical Assistance:
Discussed with the Director and Lead Teacher staff record requirements, child record requirements, posting requirements. and mixed-age grouping.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am to 11:15am with the Director and Lead Teacher. There were 18 children in care, ranging from three years to five years of age, supervised by two staff. The children were observed having free play in centers. Five children?s records and six staff records were reviewed. The Center has two staff with current certification in CPR and First Aid, and three staff current in Daily Health Observation Training. There were no medications on site for children but the center has two staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The most recent Fire Inspection on file was dated 3/28/19 and the most recent Health Inspection on file was dated 8/7/19. Standard #: 22VAC40-185-60-A
Description: Based on review of five student records, the center did not obtain all required documentation for each child's record. Evidence: The record for Child A did not contain documentation of name, address, and phone number of a second designated person to call in an emergency if a parent cannot be reached.

Plan of Correction: We will ask the mother for that information this afternoon.

Standard #: 22VAC40-185-70-A
Description: Based on review of five staff records, the center did not obtain all required documentation for staff records. Evidence: The records for Staff A (with a start date of 9/3/03) , Staff B (with a start date of 4/25/19), Staff C (with a start date of 10/2012), Staff D (with a start date of 4/29/13), and Staff E (with a start date of 10/12/16) did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: We believe documentation of references should be in the files at another location so we will attempt to get them from there. If we are unable to locate them, we will conduct reference checks.
