Comments:
An unannounced Monitoring Inspection was conducted on January 21, 2020 from approximately 1:15 pm until 6:00 pm. Upon entrance there were 118 children and 15 staff present with children. Additional staff were on site for program and administrative purposes. The record review sample consisted of 11 children's records and 6 staff records. Ten medications were reviewed. Children were observed during afternoon routine, to include resting and eating snack. Infants were observed freely playing, sleeping and eating.
The results of the inspection were reviewed and verified with the Center Director during the exit interview. Areas of noncompliance are noted on the violation notice. If you have any questions, contact your licensing inspector at (757) 404-0568.
The inspection was amended on 1-30-2020.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review of 11 children's records and staff interviews, the licensee did not ensure that each center shall maintain and keep at the center a separate record for each child enrolled which shall contained all required information.

Evidence: The records for child #6 did not include the employment phone number for one of the parents listed. The record for child #9 did not include a 2nd emergency contact name, phone number and address.

Plan of Correction: Form was updated with a complete second emergency contact by parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence : The following physical plant items were observed and pose a health and safety risk to the children.
1) In room #103, (children age range 16 months to 2 years) near the entrance door there is an area between 8 to 12 inches in length and with that has chipped and peeling paint.
2) On the playground, there is a wooden shed and at the bottom of the shed door there is an area that measured between 16 and 20 inches long where the wooden door is deteriorating, splintered and sharp. There are also 5 protruding nails at the bottom of the shed door.

Plan of Correction: The room with 16 months to 2 years, was room #103, the area was covered. The outside shed has been blocked off for staff and students until we can find someone to replace the word.

Standard #: 22VAC40-185-440-B
Description: Based on observation and staff interviews, the licensee did not ensure that cribs, cots, rest mats, and beds shall be identified for use by a specific child.

Evidence: In room #104, 11 children were observed during their designated rest period and 3 children were observed sleeping on a cot that was not identified for use by a specific child and 1 child was sleeping on a cot labeled /identified for another child.

Plan of Correction: The teacher was told to update labels on children's cots.

Standard #: 22VAC40-185-500-A
Description: Based on observation and staff interviews, the licensee did not ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence: In room #104, 8 children were observed eating snack and they did not wash their hands prior to eating.

Plan of Correction: Teacher was told the importance of hand washing was also reminded when hand washing should be done.
