Discussed USDA food serving requirements with new center staff and director.

An Unannounced Monitoring Inspection was conducted on January 30, 2020 from approximately 8:25 am until 1:15 pm. Upon entrance there were 70 children and 12 staff present with children. Additional staff were on site for administrative and program purposes. The record review sample consisted of 7 children's records and 4 staff records. Children were observed preparing for and getting on the bus for school, eating snack, playing in centers to include dress up, books and toys. Infants were observed freely playing, eating and sleeping. Violations from the previous inspection were reviewed and corrected.

Standard #: 22VAC40-185-40-J Description: Based on documentation review and staff interviews the licensee did not ensure the injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.
Evidence: The most recent updated injury prevention plan was from 2018 injuries. The center director confirmed that one had not been developed for this year.Plan of Correction: The center director will develop the plan by the end of the day.
Standard #: 22VAC40-185-270-A Description: Based on observation and staff interviews, the licensee did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: On the toddler playground, inside the wooden house there are five areas where the wood is splintering and cracked. There is a sharp piece of metal protruding from the wood frame inside the wooden house that measures between 1 1/2 inches to 2 inches long.Plan of Correction: The center director will have it repaired within two weeks. The metal will be removed today.
Standard #: 22VAC40-185-500-A Description: Based on observation and staff interviews, the licensee did not ensure all children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence: In the following classrooms not all children washed their hands after eating snack.
1) In the Two-B room (ages 1 to 2) there were 4 children observed eating snack and then immediately the children got up and started to play in the circle time are with toys.
2) In the Pre-K-A (age 4 ) 4 children were observed eating snack and then immediately got up and prepared for and went outside.Plan of Correction: The center director will talk to staff today and retrain all staff on washing hands by tomorrow.
