An unannounced monitoring inspection was conducted on January 30, 2020 from approximately 11:10 am to 12:55 pm. Eight staff were present with thirty-nine children. Additional staff were on site for administrative and kitchen duties. Five children and three staff records were reviewed.

Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 2 of 5 children records have documentation of required information.
Evidence: The following was found during record review:
1. The record for child #3 (start date January 27, 2020) did not have documentation of any known allergies and previous programs attended.
2. The record for child #4 (start date January 6, 2020) did not have documentation of previous programs attended.Plan of Correction: Director will get the information from the parents today.
